Below please find the attachments for your daughter’s summer reading lists, book order forms,

gym suit order form and medical forms.  Please note that there are two medical forms. 

One is the regular form and the other is a sports medical form.  If your daughter intends to try  

out for any sport team, you must submit a completed sports medical form.
